Minimum three (3) years of healthcare or related experience
Experience with care coordination and resource coordination
Knowledge of medical terminology
CHW - ADHS Certification required
Bilingual preferred
Skills
Strong interpersonal, communication, and interviewing skills
Critical thinking and problem-solving abilities
Organized with the ability to manage multiple patient records
Team-oriented with strong adaptability and flexibility
Comfortable providing health education and coaching
Proficient with email, Microsoft Office, and patient record systems

Standard schedule: 8:00 AM - 5:00 PM, with flexibility as needed A Community Health Worker (CHW) is a nonmedical health worker who serves as a liaison between health, social services, and the community to facilitate access to services, improve the quality and cultural competence of service delivery, and facilitate coordination of services to improve medical and behavioral health outcomes. The Community Health Worker helps eliminate disparities in care to make healthcare systems more equitable by providing health system navigation and resource coordination. By helping with the coordination of care and providing health promotion, this role assists patients with complex conditions and manage office visits, testing services and treatments.
Education
- High School diploma or equivalent (Required)
- Associates in a related field or a combination of education and work experience. (Required)
- Maintain minimum education and training qualifications for certified Community Health Workers (Required)
Experience
- Three (3) years of experience in healthcare or related field. (Required)
- Experience with medical terminology. (Required)
- Experience with care coordination and resource coordination. (Required)
Certifications
- Current BLS certification for Healthcare Provider from the American Heart Association. (Required)
- Community Health Worker Certification through Arizona Department of Health Services (ADHS) (Required)
Special Skills
- Bilingual (Spanish) (Preferred)
- Interpersonal skills to promote positive interactions with patients and health care professionals (Required)
- Interviewing skills to assess patients’ needs (Required)
- Organizational skills to maintain accurate records for multiple patients (Required)

- Strong oral communication skills to convey health information in an easy-to-understand manner (Required)
- Health promotion and/or coaching to provide information and training to members that enables the member to make positive contributions to their health status (Required)
- Must function well in a team environment (Required)
- Providing health education and training, including services to train and/or increase the member’s awareness of methods and measures that have been proven to be effective in avoiding illness and/or lessening the effects of an illness. The content of the education shall be consistent with established healthcare standards. (Required)
- Computer skills to use email, word processing, spreadsheet and patient records management software (Required)
